Haiti - FLASH : Documents required for the Vitem 3 humanitarian visa for Brazil 02/11/2024 07:35:01
Required documents : - Passport valid for at least twelve (12) months on the day the applicant's file is submitted, with at least two (2) blank pages; - Two (2) recent passport-size photos on a white background (5 x 5 cm); - Criminal record extract or police certificate issued by the Central Directorate of the Judicial Police of Haiti (DCPJ) (valid for 6 months) or by the Courts of First Instance (TPI) (generally valid for 3 months), for adults; - Reservation of the plane ticket (travel itinerary); - Extract of birth certificate issued by the National Archives, duly legalized by the Public Prosecutor's Office, the Ministry of Justice and Public Security, and the Ministry of Foreign Affairs and Worship. Birth certificates issued by the Courts of Peace will not be accepted; - Original receipt for payment of the BVAC fees, in the amount of 60 US dollars per application, to the IOM account at SOGEBANK # 2616027989. VITEM 3 implies that the applicant is an adult.
